Re: Patient Charter for Home Parenteral Nutrition

As the Chair of the Stakeholders group for the National Framework for Home Parenteral Nutrition in England, I am pleased to be able to share the Patient Charter for Home Parenteral Nutrition with you.

This document has taken time to complete as we wanted to ensure that it contained all the pertinent information for you. It has been written by the HPN Stakeholders Group -a multidisciplinary panel of experts commissioned by NHS England, including clinicians, nurses, pharmacists, dietitians, patients and commissioners.

The idea of the patient charter is to ensure that you, the patient or carer, has clear information of the service you can expect from any homecare company providing this service, specifically the supply and delivery of the feed and equipment, and what you can expect from the nursing service if your hospital have arranged for you to receive nursing. It also explains the responsibilities of the hospital that prescribes your parenteral fluids or feed to you and the provision of this service. Finally, the document sets out the requirements of the patient or carer to the homecare company, nursing service, and the hospital, as the provision of this therapy is a partnership between all parties. All of this is in line with the NHS Constitution.

I do hope that you feel this document is clear on your rights and responsibilities when receiving home parenteral fluids or nutrition at home.

I do wish to thank PINNT (www.pinnt.com) for getting these printed so we can get them sent out to you as soon as the document was printed. We are also liaising with the homecare companies for them to also have copies printed. Your hospital will also be sent an electronic copy which they will be able to share with you.

If you do have any comments on this document, then please contact your HPN prescribing centre.

I wish you all well.
